In consideration of the payment of $200.00 or $250.00 (circle correct one), the receipt of which is acknowledged, ownership is transferred to the undersigned adopter who assumes all risk and financial responsibilities of ownership. The adopter releases Sheltie Rescue of Georgia, Inc., from any liability associated with any illness of the dog or damage, accident or injury hereafter caused by said dog. The adopter agrees that Sheltie Rescue of Georgia will not in any way be held responsible for the conduct of this dog after its adoption. The adopter understands that some or all of the information provided on the Adoption Agreement regarding said dog may have been received by Sheltie Rescue of Georgia, Inc., from a third party. The adopter understands that Sheltie Rescue of Georgia, Inc., does not warrant the accuracy of such information. In addition, if the dog was found wandering or lost, the adopter acknowledges that Sheltie Rescue of Georgia, Inc., has no knowledge of the prior owner of the dog and that Sheltie Rescue of Georgia, Inc., has made every effort to locate the owner. Additionally, if the prior owner is located, he/she may contact the adopter at any time with regard to retaking the dog. If this occurs, the only remedy the adopter has is that Sheltie Rescue of Georgia, Inc., will provide the adopter with another dog, if and when one is available and the adopter so desires, at no additional cost to the adopter. If, at any time, the adopter desires to relinquish custody, or Sheltie Rescue of Georgia, Inc., demands its release for failure to comply with this agreement, the adopter agrees to return said dog to Sheltie Rescue of Georgia, Inc., making no charges of any nature for licensing, care, food or other services or items provided by the adopter. The adopter agrees to pay any reasonable attorney fees and court expenses if Sheltie Rescue of Georgia, Inc., must bring legal action against the adopter to enforce this Adoption Agreement. The adopter understands that any sum of money they have given to Sheltie Rescue of Georgia, Inc., is to be used for work in caring for this and other Shelties and is not refundable except as provided in the Terms of Adoption.
TERMS OF ADOPTION
Return: If the dog is returned to SRG within 2 weeks of the above date the adopter will receive a full refund of the adoption fee, but after 2 weeks no refund will be given. It is the sole obligation and responsibility of the adopter to return the dog to the SRG representative of our choosing and to no other individual or rescue. If, at any time during the dog's life, the adopter is unable to keep the dog for any reason, SRG must be contacted and the dog will come back to SRG with the understanding that there will be no refund of any moneys.
Transfer of Ownership: The dog may not be transferred to any other person for any reason. If the undersigned adopter is forced to relinquish custody of the dog at anytime for any reason, said dog will be relinquished only to Sheltie Rescue of Georgia, Inc.
Refund: There will be no refund or reimbursement for any expenses incurred by the adopter for the above designated dog, even if returned to Sheltie Rescue of Georgia, Inc.
Health Program: The dog will be cared for humanely, including a high quality brand food (not a grocery store brand), shelter, water and veterinary care and will be licensed in accordance with the laws in the Jurisdiction in which the dog resides with the adopter. The dog is to be an inside companion and be exercised in a fenced area or on a leash and is NOT to be free to roam nor will the dog be physically restrained to any inanimate object (trees, cable run, etc.). Heartworm preventative and annual heartworm checks MUST be maintained as well as annual or 3 year rabies vaccinations, and other annual boosters or titers (tests of antibody levels in the blood to determine how much immunity the dog carries against a specific disease), as recommended by your veterinarian. Interceptor is the recommended heartworm preventive to be used for Shelties and Collies due to adverse effects of the Ivermectin chemical used in Heartguard.
Loss: If the dog is lost or stolen, the adopter will notify Sheltie Rescue of Georgia immediately. By maintaining the Rescue Tag on the animal, which is a requirement of this contract, Sheltie Rescue may be notified that the dog has been found and will assist in returning the dog to the adopter. Should the tag be lost, the adopter is to notify SRG so that a replacement tag can be obtained.
Reservation of Rights: Sheltie Rescue of Georgia, Inc., reserves the right to enforce this contract in order to protect the welfare of the dog. It is understood that Sheltie Rescue of Georgia, Inc., may examine and may make inquiry about said dog at any time. If the terms and conditions of this Agreement are not upheld, Sheltie Rescue of Georgia, Inc., reserves the right to terminate this Agreement and to reclaim the dog.
Return Check Charge: There will be a $15.00 charge on all returned checks submitted by the adopter.
I hereby acknowledge that I have read and understand this Adoption Agreement and agree to abide by all of the rules and regulations set forth within this document. I pledge that I will keep the dog described herein as a part of my family.
